Iakiv Vitaliiovych Fishchenko is a professor, Doctor of Medical Sciences and physician of the highest qualification category. His specialisations are orthopaedics and traumatology, neurosurgery and spine surgery.

His practice covers surgery for disc herniation, lumbar and cervical stenosis, spondylolisthesis, scoliosis and vertebral fractures, as well as stabilisation and revision procedures. Another area is surgical correction of pectus excavatum.

Chief Research Fellow in the Department of Spine Surgery at the National Institute of Traumatology and Orthopaedics of the National Academy of Medical Sciences of Ukraine. He also works and performs surgery at MedProfi private clinic. He combines clinical practice, research on treatment outcomes and teaching endoscopic surgery.

Surgery for lumbar disc herniation

Professor Iakiv Fishchenko performs surgery for lumbar disc herniation, including sequestered and migrated herniations and recurrent disc herniation. The aim of disc removal is to relieve pressure on the nerve root. The approach is selected according to symptoms, examination and MRI, taking the outcome of previous treatment into account.

Techniques: Main specialisation: biportal endoscopic discectomy BESS / UBE. Indications and the surgical approach are determined individually.

The professor's research examines the outcomes of endoscopic discectomy and comparisons with microdiscectomy. The publications describe specific patient groups and do not predict the outcome of an individual operation.

Surgical treatment of lumbar spinal stenosis

Surgery for narrowing of the spinal canal that compresses neural structures and may limit walking. Lumbar stenosis is one of the central areas of the professor's clinical, research and teaching work. Planning involves assessing whether neural decompression alone is sufficient or whether stabilisation is also needed.

Techniques: Biportal endoscopic decompression BESS / UBE; combined with stabilisation when indicated.
Experience and related materials

The professor is a co-author of studies on decompression for lumbar stenosis. These evaluate symptoms, functional status and postoperative investigation findings; the data relate to the study populations.

Surgery for spondylolisthesis

Surgical treatment of a displaced vertebra, with assessment of segmental stability and nerve compression. The professor considers decompression, stabilisation or their combination according to the clinical situation. Displacement seen on an image alone does not establish the need for surgery or its extent.

Techniques: Neural decompression; interbody fusion and fixation when indicated, including endoscopic techniques.

Surgery for cervical disc herniation and stenosis

Decompression, stabilisation and motion-preserving procedures on the cervical spine. The consultation distinguishes nerve root compression from spinal cord compression — cervical myelopathy. The approach depends on the level and nature of compression, stability and symptoms.

Techniques: Endoscopic decompression, anterior cervical discectomy and fusion (ACDF), and disc replacement.

Surgical correction of scoliosis

The professor corrects spinal deformities using instrumented and dynamic techniques. Surgical planning considers the shape and flexibility of the curve, trunk balance and skeletal maturity. Anterior dynamic correction without fusion is a separate area of practice with its own selection criteria.

Techniques: Anterior dynamic correction ASC / VBT; posterior instrumented correction with fusion.

Surgery for pectus excavatum

Another area of the professor's surgical practice is correction of pectus excavatum using the Nuss procedure. Planning considers the shape of the deformity, symptoms, investigation results and follow-up. His practice also includes treatment of pectus carinatum and pectus arcuatum.

Techniques: The Nuss procedure and individually selected techniques for chest wall deformity correction.

The professor is a co-author of patents for a pectus excavatum correction method and a fixation device for its surgical treatment. Related research addresses postoperative recovery.

Treatment of vertebral fractures: vertebroplasty and kyphoplasty

Minimally invasive procedures for vertebral compression fractures, including those associated with osteoporosis. The professor assesses the fracture pattern, bone quality and whether imaging findings correspond to symptoms. Not every fracture requires surgery; the technique is determined after assessment.

Techniques: Vertebroplasty, balloon kyphoplasty and SpineJack.

Spinal stabilisation surgery

Restoring the supporting function and stability of a spinal segment in cases of instability, deformity and other conditions requiring fixation. The professor plans stabilisation as a procedure in its own right or combined with neural decompression. The extent of fixation and the need for implants are discussed before surgery.

Techniques: Pedicle screw fixation, interbody fusion, endoscopic fusion techniques and combinations with decompression.

The professor's research includes a comparative study of biportal endoscopic and posterior lumbar interbody fusion. The choice of technique depends on the clinical objective.

Choosing the technique is part of the surgeon's work. Indications for surgery are determined after assessing symptoms, examination findings and complete investigations. Endoscopic techniques, stabilisation and dynamic correction address different problems.
